Passport Program

Passport provides funding for services and supports so adults with a developmental disability can:

• take part in community classes or recreational programs
• develop work, volunteer and daily life skills
• hire a support worker
• create their own life plans (this is called person-directed planning) to reach their goals
• get temporary respite for their caregivers

Note: You must be registered with DSO to be eligible for Passport funding

Ontario Disability Support Program (ODSP)

ODSP offers:

• money to help you and your eligible family members with living expenses, including food, and rent
• health benefits, including prescription drugs and vision care
• employment support to help you find and keep a job or advance your career

CAMH Adult Neurodevelopmental Services

• Intended for adults 16 years and older with a documented diagnosis of an intellectual disability and/or autism presenting with mental health concerns or behaviours that challenge such as severe aggression, self-injurious behaviours (SIB) or severe persistent repetitive behaviours.
• Physician referral required through the referral form

Surrey Place Adult Psychiatric Clinic

• specialized consultation-only service for diagnosis or medication review. This service is available to adults aged 18-45 with an intellectual or developmental disability or autism spectrum disorder (ASD).

Surrey Place Psychology for Adults

• Services include:
  • Cognitive and socio-emotional assessment
  • Parent support and consultation
  • Diagnostic assessments
  • Individual and group psychotherapy

Bereaved Families of Ontario

• An inclusive community fostering hope and healing for individuals grieving the loss of a loved one. 

Community Integration through Cooperative Education (CICE)
